The rink is open and we are excited to have you on the ice!

HOWEVER, the pandemic is still with us, and there are precautions that still need to be taken during the time of COVID-19.

For your safety and the safety of others, please take a moment to complete the health questionnaires below BEFORE you leave your home.

Then upon submitting our form below you will receive an email copy of the details you’ve submitted. Please, either print or bring your mobile device with the email on it, to show to our staff on duty at the rink.

COVID-19 Health Screening Questionnaire

If you are visiting the premises on your own, click here for the correct form.

    Please fill in details for your child below.
    Kindly note that adults will not be allowed into the rink. Your child's temperature will be checked at the door and we reserve the right to restrict entry for the safety of everyone in the rink. Entry will be denied should you or your child show any of the symptoms listed below.
    Date of birth
    Is your child experiencing, or have they experienced in the past 14 days, at least one of the following symptoms:
    • Constant Cough
    • Sore throat
    • Shortness of breath
    • Fever (a temperature of 37.3°C or higher)
    • Constant Chills
    • Body aches including headaches
    • Loss of taste/smell
    • Nausea, diarrhoea and/or vomiting
    Has your child been in close contact or in a closed space with any COVID-19 confirmed cases within the past 14 days?
    Close contact refers to being in contact with:
    - a person who was diagnosed with the COVID-19 coronavirus virus even if they did not exhibit symptoms,
    - had face-to-face contact within 1 metre of a COVID-19 positive person,
    - or was in a closed space for more than 15 minutes with COVID-19 positive person.

    COVID-19 Entry Terms and Indemnity

    1. By completing this COVID-19 screening questionnaire to enter the premises, I confirm to the best of my knowledge that
      1. Neither my child nor myself are infected with the COVID-19 virus,
      2. We have not been in contact with any person who has been infected, and
      3. We have not travelled to any destination that is considered a high infection rate area.
    2. I also agree and give express consent to the agent(s) to obtain, process and further process my Protected Personal Information for any lawful purposes relating to its COVID-19 preventative measures implemented at the premises. I understand that my Protected Personal Information includes at least, my full names and surname, identification number, address, and telephone number and if applicable at least, my child/children’s names and surname and date of birth/s. The obtaining, processing and further processing concerned includes at a minimum to:
      1. Record my Protected Personal Information;
      2. Meet any lawful obligation imposed on it;
      3. Furnish my Protected Personal Information to any authority and entity for purposes of giving effect to the promotion of COVID-19 restrictions and preventative measures;
      4. Performing any analyses to identify potential risks, persons and trends in relation to COVID-19.
    3. Furthermore, I agree and consent to the above mentioned retaining my Protected Personal Information for such periods as deemed necessary for its legitimate business purposes and record keeping protocols as required by law.
    4. I accept that the above mentioned take reasonable steps to safeguard Protected Personal Information in its possession.
    5. I hereby indemnify and hold harmless Tri Ice Sports Holdings (Pty) Ltd; the Central Gauteng Figure Skating Association; South African Figure Skating Association; all coaches and any other agents against any liability, damages and/or legal action whatsoever and howsoever arising, including negligence on the part of the above mentioned or agents, and the above mentioned shall not accept liability for any damages or any type of loss suffered by me.
    6. All persons entering and /or utilising the premises and/or any facilities, do so entirely at their own risk and shall have no claim for personal injury or damages of any nature whatsoever against the owners of the premises, or Tri Ice Sports Holdings (Pty) Ltd, the Central Gauteng Figure Skating Association, South African Figure Skating Association, regardless that any such claim may have arisen as a result of the negligence or gross negligence of said owners and the above mentioned.
    7. Central Gauteng Figure Skating Association and/or their agents reserve the right to refuse me or my child access to the premises should we present any signs or symptoms in relation to the COVID-19 virus, or in the event that I or my child are not fulfilling the purpose for which we entered these premises, or for our undesirable conduct within these premises for purposes of promoting COVID-19 preventative measures.
    8. In the event that the above mentioned makes any indulgence or deviation of these terms and conditions in favour of me or my child, then such indulgence or deviation shall not be misinterpreted to be a breach or waiver of these terms and conditions or any rights of Central Gauteng Figure Skating Association.
    9. The above clauses are severable from each other. In the event that any clause or part thereof is unenforceable in law, then that clause or part thereof shall be severed from these terms and conditions, and the remaining clauses and parts thereof shall remain in force.
